Generally when I see this in the NCPA wizard it's usually a FW/web gateway/proxy/IPS device/security software preventing access to it, especially if you don't see it in the access logs. That means it's not even making it to httpd so likely blocked in-between. Okay so it is, just hard to find in all...

Having the same issue with newest Nagios XI build on Ubuntu 20.04.2.
Once I hit next, it goes connection reset. Nothing in other_vhosts_access.log when this happens. No proxy other than being VPN'd into work (thanks covid). Everything else so far seems to work fine.
