If it's of any use 192.168.5.4 would be on the network our developers use. I could hypothesise that you need to configure the component this is done on the management screen where it's uploaded.

I've seen this when the hostgroup all is deleted or not every host belongs to it. Check the URLs being passed for some clue to the actual filtering being performed.

You can also use Web Transactions. Though I feel that would be overly complex a solution.
The benefit for having this as two checks is that you are directed to what part is acting up, rather then having to decide/discover what request is not happy today.
