Found it!!

Lighttpd nagios.conf file had this:

setenv.add-environment = ( "REMOTE_USER" => "user" )

Removed it and all sweetness.

This is very odd ... it just seems to ignore the contact_groups setting for the 2nd group.

I will report back with my findings - hopefully positively.
