Email alerts for warning and SMS alerts for critical state

Hi,

I will appreciate if somebody can give me a working example to set up Email alert for warning state and SMS alert for critical state of a service. For example MySQL Slave IO and SQL thread is critical and MySQL Slave lag is in warning state as per the predefined threshold.
