I did restart Apache but no change in result.

@mcapra was able to get it working with the following apache configurations (based off of yours). (see attached)

As we cannot control everything at play here with apache, I would take a look and diff what's in the zip to see if you're missing a setting somewhere, or if something has been done differently. It could even be a permission on the folder Nagios is trying to present to apache depending how things are setup to run in your environment.

I spun up a new VM and started over and Nagios appears to be working now. Not sure what the issue was/is but this solution works.
