It appears there is still an issue with SNMP running on the switch. Can you show us the output of show run from it? If not the whole output, anything that pertains to SNMP.

Can you post the snmp-server settings and the access-list from the switch?
What is the IP address of the XI server?

Take a look at this link and verify that the settings on the switch are correct.

Just to clarify, is the permit IP the one that Nagios XI uses?
