Clint,

Had the issue again Friday last week and tested your suggestions. Sending commands directly to cmd.cgi also didn't work. Rebooted the server afterwards and the issue didn't re-appear for now.

It's failing getting to the nagios process. Let's enable debugging under the CCM > CCM Admin > Core Configs > nagios.cfg. Set the debug_level line to 'debug_level=772' and increase the max_debug_file_size to 10MB with 'max_debug_file_size=10000000'. You'll need to apply the configuration to make sure the service restarts and the changes apply. The debug file will be written to /usr/local/nagios/var/nagios.debug. We'll want to gather this as soon as the problem is noticed. You may also need to keep an eye on the file and make sure it doesn't turn over too frequently to make sure we get good info. Increase max_debug_file_size if needed.
