I think we may need a profile.zip from Admin -> System Profile

Also, does it happen on every apply?

A lock file would not explain how connecting the low-information object to a template corrects the behavior.

I have had apply not finish two times when I forgot to connect a low-information object to a template. Upon connection to a template the apply then works. It is working now.
