I thought I had posted this earlier, but I'll try again.

I have seen this problem. Every time it is the result of the queue length in the SMTP server being set to 15 minutes to an hour. Since you say it is 5 - 15 minutes, it looks like it is set to 15 minutes on the smtp server.

If the smtp server is running Sendmail, run sendmail -q on it and see if that gets you an immediate email. If so, the bottleneck is not nagios but your smtp server.

Thanks for the log. However, it doesn't help a lot without knowing when the Nagios XI server is sending out email. You should compare timestamps.

Can you go in to Nagios XI's notifications and screen capture the notification that you are having issue with?
Also, we need you to post the received email with the full headers displayed.

Can you provide us the logs from your Nagios XI system, so that we can compare the timestamps? /usr/local/nagios/var/nagios.log - this will show when they are fired on the Nagios XI system.

We will also need to see the corresponding logs with time stamps from your SMTP server. Please post those as well.

In addition to trgiep's response, are you relaying through Exchange? A mail gateway? Postfix/Sendmail? Other?
