Nagios Version:2012R1.7
Centos Version 6 64 bit
I am trying to remove the username and password from the SMTP authentication section under "admin"->manage email settings and it keeps returning back the old usernames and passwords I blanked out. Is there some way to clear this out?

Have you tried clearing the browser's cache?

Amazingly no. I'm going to kick myself if it is that simple though.

Yeah, I cleared my cache, closed the browser and reopened it. The old username and password are back. The only thing that seems to remove the username and password are when I put a " " in for the username and I blank out the password.

I'm sorry - when I said "cache", I meant clearing everything (including cache). That's what I do...

Most probably you will need to clear "Form & Search History" but if you use Firefox, you can try the following:
Go to Tools->Clear Recent History, select "Everything" from the "Time range to clear" drop-down menu, select all of the tick boxes under "Details", and click on "Clear Now". Close the browser, reopen it, and try again.

Yeah I definitely cleared my browser cache completely and reopened the browser. The odd part is that I added a username for as the smtp user. I'm don't think this is a cache related issue in this case. I'm looking further into the issue, but let me verify a few things first on my side and get you some clear duplication instructions.

Do you have the login credentials for nagios saved into your browser? I found that with firefox, because I have my credentials saved and we use the same form field names that it autofilled. With chrome it did not seem to have this same behavior. Blanking out regardless of browser and submitting did seem to clear the credentials despite the browsers autofill on reload.
Does this seem like the same case to you?

In this case I do have them saved in the browser. I am going to clear those out to see if it is still taking them. I'll let you know tomorrow about the results.
