SNMP Health monitoring

HI all..
I have some custom applications/services running in windows environment. These services are publishing SNMP statistics for their health and other vital status. I am looking for a way to monitor my these services through nagios core.
I have custom made mib also for my these applications.
Please suggest how can i monitor health of these applications.
