Nagios XI Alerts to PagerDuty Stopped

Having an issue with Nagios XI randomly stopped alerting to PagerDuty.
We had an outage that we were blind too, as well the box was unavailable but Nagios Checks showed OK for hours.

Need the steps to validate what happened, why and that we can rely on this to work next time.
This was working fine before. We have not made changes to the XI config for alerting/notifications in months on this XI instance.

1. What are the logs to check for that time period to see?
2. Are there updates for PagerDuty to apply?
3. What next to ensure this working?
