Nagios Notifications

Hello,
Can someone point me to the documentation that describes how the notifications are formatted? Our notifications use the following parameters when a service notification goes out:
Service:
Host:
Address:
State:
Gives date and time
Notes:
Additional Info:
The Notes above is the information that is defined in the host notes parameter. I would like to have it use the service notes parameter if there is one. How can I do that?
