not able to add services

Hi Team,
We are not able to add more services in our host
hostname is ivpcloud.com and we want to add more urls
https://ivpcloud.com/BBGAuthServiceProd/
https://ivpcloud.com/CSMServices/
https://ivpcloud.com/CloudPMSUAT/
https://ivpcloud.com/PnLMonitorPriceModuleProd/
in same host when i add url in run monitoring wizard > website it will added successfully but its not showing in services,

What version of XI are you on?
XI > Admin > System Profile > Download Profile
Please include the zip file in your response. You can PM myself or other support personnel if you have privacy concerns.

We have 30 above urls are running in single domain and we want to add all of this urls in single host and could you please us know the email address so that i send you the profile.zip

It looks like you have crashed tables, I would take care of this which may help -

160406 15:37:53 [ERROR] /usr/libexec/mysqld: Table './nagios/nagios_servicestatus' is marked as crashed and should be repaired
You can do so my running the following script -
/usr/local/nagiosxi/scripts/repair_databases.sh - then try to use the wizard once again. If the issue persists, please show us the screenshots once again. (the ones in your initial post still are not working.)
