For the last couple of releases when I click on get update, the window is blank. (sidenote: Proxies are turned off on Nagios so no proxies required). What am I missing to update through the gui?

What I noticed later after posting this is that there are two buttons that blink for only a moment... When you click on 'check for updates' (under the Admin panel) A blue button 'Check for updates now' and a green button 'Upgrade to latest version'. Any ideas? I've cleared cache and tried both in IE and Chrome to no success.

Try an upgrade and then immediately run tail /var/log/httpd/*_log. Then just copy and past the output into individual code blocks for each log (or one big code block).

You can create the code block using the "Code" button above the input window. It's best to press the "Code" button first, otherwise you'll have to get rid of the end of the block and then put it in the proper place.

I'm surprised you see no other issues, but run
I'm assuming this is CentOS6 since you have Apache 2.2, that second command should work if it's not CentOS6, but it might redirect you to systemctl

Please let us know if that does not resolve things for you.
