setup alerts dynamically based on the monitored host name?

Hello,
We are using Nagios core to monitor network devices and servers. I have alerting setup to email me when certain servers go down and come back up.
Is it possible to configure multiple email addresses to receive alerts dynamically based on the host or computer name being monitored?
example: an alert on computer name CORP1-41YAC, would go to a POC in Region 1, an alert on computer name CORP4-FHL32, would go to a separate POC in Region 4, etc.?
