New XI install - apply configuration hangs

Hello -
We have a new Nagios XI 5.4.3 install and are unable to complete the step to apply configuration. I tried adding a Cisco switch via the Config Wizard and a simple host with icmp health check. Both just sit and spin after applying, with the message "Waiting for configuration verification..."
cmdsubsys.log shows these messages at the time:
/usr/bin/wget --save-cookies nagiosql.cookies --keep-session-cookies http://localhost/nagiosxi/includes/components/ccm/ --no-check-certificate --post-data 'submit=Login&hidelog=true&loginSubmitted=true&backend=1&username=nagiosxi&password=*********' -O nagiosql.loginNAGIOSQL LOGIN FAILED!
OUTPUT=/usr/bin/wget --save-cookies nagiosql.cookies --keep-session-cookies http://localhost/nagiosxi/includes/components/ccm/ --no-check-certificate --post-data 'submit=Login&hidelog=true&loginSubmitted=true&backend=1&username=nagiosxi&password=*********' -O nagiosql.loginNAGIOSQL LOGIN FAILED!
RETURNCODE=2
Any ideas of what we should check for? Looks like a database authentication failure, but I don't know where to correct it.
