Largely, this is not really nagios question, but a networking question. We'll do what we can to help though.
Typically nailed up VPNs are treated like local network segments. Not always - but typically. If that's the case in your environment there is nothing for you to do.
Otherwise, you'll need to allow access inbound to the network on 5666 and 12489 (specifically for NRPE and nsclient, assuming you are using those). If you need to use other monitoring agents such as WMI and/or SNMP that list will need to be expanded.
If users on the other side of the tunnel need access to the Nagios web interface, then you'll also need access inbound to your network on port 80.

What have you tried? That will tell us where we need to start.
Can you PM me your Profile (this will give me some clue as to what you've tried)? You can download it by going to Admin > System Config > System Profile and click the ***Download Profile*** button towards the top. If for whatever reason you *cannot* download the profile, please put the output of View System Info (5.3.4+, Show Profile if older) in the thread (that will at least get us some info). This will give us access to many of the logs we would otherwise ask for individually. If security is a concern, you can unzip the profile take out what you like, and then zip it up again. We may end up needing something you remove, but we can ask for that specifically.
After you PM the profile, please update this thread. Updating this thread is the only way for it to show back up on our dashboard.
