The root cause for this issue may be Nagios XI software versions for our RHEL system. Nagios XI version 5.5.3 worked. after three (3) re-installations, I think this issue has resolved. ( I dropped the nagiosxi database and user )

Still do not know the root cause ...

I tried to install the action component. The UI said installation was successful but the action component does not show in item list.
