Creating Graphs for Custom Check Scripts

Hi,

I am running Nagios XI 2012R2.9. Is there detailed documentation on how to create graphs from custom check scripts? I believe I am almost there. I have Performance data being written from the check, but I have not been able to find what else needs to happen. I have searched through support and found information on performance data, but nothing to tie it to graphing.

Any help would be appreciated.
