notification disabled ignored?

I'm running Nagios XI 2012R2.9 and have a service and host marked with notification disabled. Nevertheless when the service failed, notifications did occur and were also escalated. Have you seen this before?

Can you post a screenshot of the notifications log in XI showing that the notifications were sent?

Here is the CSV report (much easier to paste into an email). Have you seen this problem in the 2012 release or the 2014 release? We are in the process of upgrading to 2014R2.0 this month.
