Setting group read permissions recursively on the html directory seems to have resolved the issue, and I have been able to complete the upgrade wizard and the web UI is now accessible.
When logging in I don't see the "complete upgrade" option. Clearing cache doesn't make a difference. It reports I'm on the latest version but also reports that an upgrade is available.

I still had a issue with the earlier ghost config. I was able to get at the nagios core config manager and fix the config, after which I reran the installation and now am no longer showing a need to upgrade. I still didn't see anywhere that I need to finish the upgrade, but at least it's not complaining that it's out of date.
