MSSQL Query Wizard input field confusion

Has anyone noticed problems with the prompts for the result thresholds for the MSSQL Query Wizard? On the current version (1.51, I think) the prompts and descriptions see a little wonky:

I tinkered with the PHP code a bit and it *looks* better to me, but I don't have an MSSQL database available to verify the changes.
I've attached the modified wizard if anyone wants to take a look at it.
(I don't know what the mssql_query.inc.php.patch file does so I didn't touch it.)

I'll have a dev take a look at this and see if they can clear that up. Thanks for the submission!
