How to do make an update of switch's interfaces ?
I would like add only ports Who are not already detected.

You'll have to rerun the wizard and select only those ports.
The original ports set up should not be removed. The switch and router wizard is a tricky one. It scans the device, creates mrtg configs for each port you select, then creates a Nagios service to monitor each of those MRTGs... 3 steps instead of the typical 1.

This method is somewhat laborious when we have multiple interfaces that use the same name. It forces a re-check each name and eventually return the same name at the first scan.
On the switch stack , for example, 48-port it can quickly be source of problem (especially with the Port-Channel).

We have a feature request open specifically targeting enhancement of switchport naming stuff. You won't be able to see the related threads unless you're a customer as it's in the customer only forums. That said I can take any feedback you might have and put it into the feature request notes.

I wish there is a scan function that features only interfaces that are not already in monitoring.
It should not be based on the name that it can be changed, but rather on the index of the interfaces.

So that information is noted in the feature request. The normal disclaimer is that features are implemented based upon customer and to a lesser extent community interest. If you have a more immediate need for code changes to be implemented, custom development quotes are available, just contact sales.
