I've d/l'd and installed the latest .ova to two VMs in my environment as follows:
ESXi 6:
Deploy as VM from .ova.
Log into the new VM via console.
Assign Static IP Address & network config.
reboot
yum update (apply all)
launch browser from my desktop (Chrome & Firefox -- latest versions) using newly set IP address.
Attempt to continue for cluster installation. Am never presented with anything to enter cluster information.
Sales support believes that my browsers do not allow javascript. I've sent them screenshots proving that javascript is enabled. I also have no problem with javascript on other sites. (I mean javascript being disabled is VERY noticable -- most websites would not work.)
I've logged into the systems as root and have even run the ~/scripts/cleandemo.sh to begin the setup process anew to no avail.
I've searched through the forums and "documentation" without any luck.
Can someone please provide the CLI steps to initialize and setup the cluster?

I do believe it's javascript, as that's the only way I've been able to recreate your issue. It might not be at a browser level, though. The only thing stopping you from running through the install is how NLS transitions to the 'Add Instance' section. (which uses JS)
Using Firefox, please right click the 'Final Installation Steps' page, and click 'Inspect Element'. Now, click 'Network', and then 'JS' below it. Refresh the page, and attach a screenshot of what's appearing. This should help to see what's going on.
