sending emails twice

Please help. Nagios is sending same alert notification twice. Sometimes it also send these alerts thrice. I cannot pinpoint what is causing this multiple email sending. Please help. Thanks.

Sorry for not getting back to you sooner, our offices were closed in observance of Independence Day.
