The official documentation includes all of the cases in which HARD/SOFT states are triggered. There's nothing much shorter that is also comprehensive.
In brief:
SOFT states: When a problem is detected, but the number of times that problem has been detected is less than the max_check_attempts directive.
HARD states: When a problem is detected and the number of times that problem has been detected exceeds the max_check_attempts directive.
