After performing this in-place upgrade we received crtitial alerts on all of our Oracle hosts:

Did you use the "default" oracle commands or custom ones? The reason I am asking you is because it is possible that the "default" commands got overwritten on the upgrade, and your LD_LIBRARY_PATH and ORACLE_HOME environment variables are wrong. Maybe the client version (in the commands) changed from 12.1 to 11.2...
Can you go to the CCM > Commands, type oracle in the search bar, hit "Enter", and show us a screenshot of the page?
