After loading approximately 1200 hosts with services using a customized version of the Bulk Host Import wizard, it stopped loading services and will now only load the host? Since this particular wizard was customized, I tried using the standard bulk host import wizard and it too has the same problem in that no services are indicated regardless of the chosen host for template? It acts as if I may have ran into some limit?
Thoughts, ideas, suggestions?

Can we make sure that your configuration can be applied successfully
Configure -> Core Config Manager -> Apply Configuration -> Apply Configuration

Honestly, I would have to guess then that something broke the services when it was customized...

You were probably right. I manually created a host and service for the host I was having the problem with and it resolve the problem allowing me to continue to use the bulk loader again. For the record, is there any maximum number of entries that you should paste in the window for csv input?

This is limited only by your memory and max execution time in your php.ini
