How disruptive is this update?
or better stated, how visible will this be to my end users when run?

From a UI perspective, the look and feel of the interface is the same. We've made some visual/ui improvements and added the Dark Theme mode. Overall, many patches, features and performance improvements since 5.4.4.

If you don't already have a test server deployed, I would recommend installing this on a test server to review before making any changes to production systems. Your Nagios license allows for 3 installs, primary, test, and backup.
