Exchange Connection Refused

In the host configuration do you have the actual IP address in the Address field, or just exchange2 ?

the reason I ask is that maybe it is failing because for some reason it is trying to connect to the wrong machine...

For what it is worth, the error you are receiving basically is saying that the Exchange server in refusing connections for all of the ports that Nagios is trying to connect to when this happens.

It has to be something on the server as it is not isolated to just one service/port, the screenshot you posted shows the server refusing connections for

SMTP
HTTPS
NSCLIENT

does check_ping also fail at the same time? Maybe it's a lower layer network issue.

No, ping and a blacklist are still fine.
I know BL does not go to the server, but it replies to ping, and the mail still flows as it is supposed to.

It seems as if your performance count library may have some corruption.

The following explains how to rebuild performance counter library

You'll want to take caution when following the instructions as it does require editing your registry (make a backup).

this can be resolved, I have not seen it happen again
