Authentication Not Triggering Login Prompt

I've seen something similar after upgrades. In my experience, use_authentication=1 being "ignored" is often caused by Nagios reading a different cgi.cfg than the one being edited. I'd verify the active configuration path and confirm there isn't another cgi.cfg left over from an older package or source installation.
