On the alert notifications the host is blank. Here is what the email looks like:

It gives the IP but no host name. When we add the host through the wizard and give it the IP in the wizard it is able to resolve the name. So I am not sure what is going on with the notifications. Let me know if you need more info.

I believe I can see the problem. I think that some of your hosts do not have Description set and that is throwing you message template off.
If you click the username in the top right corner of XI.
Go to "Notification Messages"
In the Host/Service Alert Subject change %hostalias% to %host%
In the Service Alert Message change %hostalias% to %host%
If you click the username in the top right corner of XI.

That fixed the issue for me, but do I need to have all users go into their profile and make these changes? Is there a way that I can make the change for all existing users and new users when they get added? I guess I am asking can I make this the default?

You can go to: Admin->Notification Management (it's under "Users"), setup a template, select the "Set these notification messages as default for all users" check-box, and click on "Update Settings".

arrrrggg...are you kidding after upgrading from xi 2011 to xi 2012 you moved the bulk notification tool into the enterprise edition, what a pain!

Were you able to do what lmiltchev suggested? The Bulk Modification Tool has always been labled as a feature of the Enterprise edition since beta:
