upgrading from 3.4.1 to 4.0.1

I compiled the default ones from: nagios-plugins-1.4.15.tar.gz

did I miss one possibly?

Maybe? Lets start by looking at the alerts for these notifications. Could you post a larger tail of the nagios.log so that we can see the alerts associated with these notifications?:
