Chris, bingo. Although I had already done so, I diffed the config files
again to make sure there were no differences. I found the problem in the
cgi.cfg file. The old file had:

physical_html_path=/usr/share/nagios/html

It should have been physical_html_path=/usr/share/nagios

This is fixed and all is working. Thanks!!!
