"Add new user" mysql error

32 bit RHEL 5.6, all updates installed
Manual install of Nagios XI 2009R1.4

When I try to add a new user, I get the following:
SQL: SQL Error [nagiosxi] : ERROR: duplicate key violates unique constraint "xi_users_pkey"ERROR: UNABLE TO ADD USERID: ()

*Update*
I ran "yum update" and updated mysql to 5.0.77-4.el5_5.5, and it works now.

Leaving this post just in case someone else has a similar issue.
