Do you already have a graphing addon installed and set up such as pnp4nagios?:

http://docs.pnp4nagios.org/

If not, you will need to set that up, or use nagiosgraph which may be already installed.

I would start by adding the host, and the service you are trying to monitor. What type of system are you trying to monitor the disk metrics on, depending on this, you will be able to follow the "How to Monitor -----" sections in this contents page:

What type of physical data performance?

Throughput, iops, latency, storage capacity utilization? Many of these can probably queried by utilizing SNMP requests to gather the data using check_snmp and routed back through pnp4nagios to generate graphs.

There may even be a plugin designed for your storage in particular. I was surprised with the availability of information on the storage systems I utilize.

If you are not familiar with it, go look at the link that I attached in my first post. Let us know if you have questions, I'd recommend using PNP4Nagios, it is quite reliable and does not have too steep of a learning curve.

That is quite a broad question, we really need more specifics from you in your questions...

PNP4Nagios will set up performance data handling for your checks, which will in turn display them on graphs if set up properly, that is all well documented in the link I sent you.
Would you please help me to configure nagios core 3.2 version.
Once again, extremely broad... I highly recommend reading through these tutorials based on what you are doing:

When you run into problems we can help you out.
