I've installed Nagios 4.0.8 on a RH6 machine and then imported some host config files from an older installation (3.2.3). When I do a change and reload the configuration with the init script, the changes aren't shown unless I restart the service.
If I check in nagios.log I can see the SIGHUP, but on the web interface nothing happens...
I believe reload has had intermittent success actually restarting nagios in the past (and maybe now). For now, you will need to most likely use restart to load a new configuration.
