Availability report .csv file only showing services

When I run an Availability Report and send via email I am able to see the PDF version for host and services and a graph without issue. However, when I run the Availability Report and send via email as a .csv file and tell it to combine both I only see the Services and I do not see labels on the columns nor hosts. Should I see labels as well as hosts and then services or am I misinterpreting what combined means here?

CSV (Combined) <--- I am using this one.
CSV (Host data only)
CSV (Service data only)
