Hello!
I am using nagios-core-3.3.1.
I see these three entries in my notification history for some particular service:
host1 PING WARNING 24-02-2012 08:03:30 mitya notify-service-by-email PING WARNING - Packet loss = 40%, RTA = 0.82 ms
host1 PING OK 24-02-2012 07:29:30 mitya notify-service-by-email PING OK - Packet loss = 0%, RTA = 0.69 ms
host1 PING WARNING 24-02-2012 07:24:40 mitya notify-service-by-email PING WARNING - Packet loss = 20%, RTA = 2.05 ms
Current state of this service is OK.
Where is final notification for service recovery (second OK)?

The last warning for this was dated 3 minutes before your post.

Anyway I explicitly stated that current state is OK.
I see this situation with lost notification almost every day.

I solved my problem by disabling flap detection in nagios.cfg.
